• Internet and other electronic network activity information, such as your IP address, browser type, browser language, operating system, the location from which you access our Services, device IDs, referring and exit pages and URLS, platform type, the number of clicks, files you download, domain names, landing pages, pages your viewed, the amount of time spent on particular pages, the terms you used to conduct searches on our Platform, the date and time you used our Services, error logs and other similar information. We collect this information generally for our own business analytics and marketing purposes as well as to better customize our content and Services to your preferences. Should you wish to opt out of our de-aggregate data pool, please submit a written/emailed request to info@goquello.com with your Name and DOB and a subject line of “Request for Removal of Aggregate Data” and we will remove your information from our aggregate data pool. This request does not affect data needed to engage in the services itself as described in this privacy policy but rather prevents us from de-identifying and using your data for other purposes.
We may share your personal information for our own business purposes in the following ways:
• Service Providers. We provide access to or share your personal information with select third parties who use the information to perform services on our behalf (“Service Providers”). I consent to the collection, testing, and insurance claim filing of my blood for the purpose of the service noted above(“SERVICE”). I release Spiriplex, Inc.(“SPX”), its affiliates and any other entity and individuals associated with this SERVICE from all liability arising from the collection and testing of my blood. I acknowledge the results for the SERVICE can be affected by many factors and does not constitute medical advice or a diagnosis of any kind. I authorize SPX, or the billing laboratory, to release all copies of my laboratory results and data to healthcare providers and facilities for the purpose of continuing and coordinating my plan of treatment, reimbursement, quality assurance, regulatory or accreditation purposes. I consent to the release of the results and data of my SERVICE (without my name being used) in clinical and research studies and reports. I hereby assign SPX, or the billing laboratory, the financial benefits I am entitled and or receive from my health insurance plan(s) or service provider(s). SPX is not a provider to government health plans (State or Federal). SPX, or the billing laboratory is authorized to release all information required for insurance claims and permit reproduction of this authorization for use in place of the original assignment.
